Conditions for obtaining service

• The dentist must be a Jordanian citizen.
• The dentist must register at the Jordanian Dental Association (JDA).
• The dentist must be a holder of a valid Professional License.
• A (120) square meters office space for the dental center to be licensed.

Procedures

1. The applicant shall visit Dental Clinics and Centers Licensing Department and submit the application and the enclosed documents to grant the license of the dental center.
2. Submit a request to conduct a field inspection.
3. Provide the applicant with a checklist of devices and tools that should be available in the dental center.
4. The applicant pays the due fees in the Financial Department.
5. The documents are verified and the application is approved. The applicant will be supplied with instructions for the necessary technical and health equipment and specifications.
6. The application is then documented in the register.
7. The application will be referred to the Director of the Health Professions and Institutions Licensing Directorate who will review and transfer it to Dental Clinics and Centers Licensing Department.
8. The application to obtain the license will be assigned for the Field Inspection Program.
9. The competent committee will carry out the actual inspection of the location to verify the availability of the necessary equipment and meeting the technical and health specifications.
   1. If the committee disapproves the location, the applicant is given a time limit to do the required amendments.
   2. If the committee approves the location, the applicant receives the approval letter signed by the Minister of Health for licensing and direct action.

Required Documents

• A copy of the registration certificate of the center.
• A copy of the commercial register of the center.
• An approval from the Jordanian Dental Association of the location of the center.
• A copy of the Professional License of the dentist (the applicant).
• A lease contract certified by Greater Amman Municipality or the Municipality.
• If the applicant (the dentist) owns the dental center, the title deed under the name of the applicant provided that the duration of their professional license is not be less than two years.
• An appointment letter for the manager of the dental center (the owner or any partner).
• Job contracts for Jordanian dentists (as a minimum), so that the number of working dentist in the center shall comply with the available dental units.
• A copy of the professional license for the working dentists in the center.
• Survey plans stating the actual space of the center.
• A licensing certificate of the panoramic radiograph from the concerned bodies.
